The contest was run by NEC.  I had to take a video of my school's lab and document the need for upgraded monitors and other multimedia equipment.  Our video was selected as one of five finalists out of hundreds.  I included the browser info because I had trouble with those.  Sorry for any inconvenience.

Our video is the one with the 2001 theme and the dog at the end.  The entry form should have info on what to fill in...  The top line is company (not important), the second and third are first and last name and the last is email address.  If you're not seeing that information I would try a different browser or refresh the page, something is obviously wrong.
